Guest Graduate Social Contract

The Grade 12 Gala formally known as PROM will be on Monday June 8th from 5:30 pm. to 10:00 pm at

Deer Creek Golf and Banquet Facility in Ajax.
Tickets will be on sale until   at $ 140.00 - payable on DHS Cash online.
( If you have trouble with the Cash online payments, please attached a cheque on $140.00 payable to Dunbarton High School attached to the permission  )
Permission Forms  for yourself and for your guest are available in the main office - front desk (side table).
Hand in back to the main office - front desk (side table):
  •  ATTACH a COPY of your PAYMENT with the meal choice to your COMPLETED permission FORMS
     
  • If you are bringing in a GUEST also add a Copy of their ID ( student ID, driver licence or health card)
            and their SIGNED permission form by a VP - ( from the school that they are attending)
